Well i have given this idea some thoughts and thought i wouldn't mind such operations available in a futuristic game, there are some problem when it involves certain race.
Jaddans: Many Jaddans are very traditional. Males are superior to Females. What if such sex changing services are available, and the Jaddan traditionalist took offense? Will they assault the hospital in protest?
Other than that, the Alteri may have a different biological makeup that differentiate a male from female, so sex changes for them, may be difficult?
Oh one final note, i saw Morgana Vli with a castration knife, so if any guys wish to do a quick sex change, just ask her...
