Do you think (maybe not for now, maybe in Live) we could have some sort of way to start a duel? I don't know if that makes any sense... like sometimes people want to duel, but they want to be sneaky and hide, so maybe they're both in hiding and nobody wants to come out because they know that they'll be attacked as soon as they emerge. Or nobody wants to say anything, because as they're typing something out, the other guy could attack them, or something like that. They had something like this in DR (challenge, I think). Just something like "X challenges you to a duel, do you accept?" to start things off.

What would be different than pvp now without duels? How would the players / game benefit?

Well, I mean... umm... just if you want to challenge someone, it'd be a way to start things off nice and fair. Crimeny, this is tough to explain, for some reason. I don't know how it would benefit the game... man, does anyone else know what I'm talking about? Does anybody else WANT what I'm talking about?

I had always thought the advantage of challenges was in the non-lethalness, not so much as a starting gun to tell both people to go, since the challenger can just have their attack typed out and ready to go while you're typing <accept challenge name> anyway.

Well, it could have a random timer after you accept.

Yeah I was thinking something like that, maybe. And you'd both have to be present and not hiding for the timer to start.
